If you use an email client such as Outlook you may simply fill in all the cell below and submit your updated information. Otherwise, you may send us an email with all of the necessary information included in the body.
To determine your network card type and MAC Address under Windows simply click "Start" >> "Run" >> type "cmd" into the box and click "OK". When the Command Prompt comes up type "ipconfig /all", your Network Card Type is the line labeled "Description" and your MAC Address is the line labeled "Physical Address" and should be twelve alphanumeric characters that look similar to "1A-B2-34-CD-E5-6F".
